Initial Appointment At your first appointment, we will provide you with some paperwork to fill out. You can make your visit even faster by downloading those forms here, filling them out and bringing them in at your appointment. We will provide an initial evaluation and consultation to suit your individual needs. Please help us provide you with the best personal care possible by bringing in the following information to your initial visit:

• A list of medications with its dosage that you are currently taking (even prenatal vitamins)
• Any and all medical or dental forms
• Any previous X-rays

Please advise our staff if you have a medical condition that may be of concern or if you require medication prior to any dental work.
